In my case, I selected the Unified Multi-Purpose ID (UMID) Card as it is the only thing I do have.

I was not certain what mobile number I provided to SSS when I first registered with them. I do not have a Savings Account Number and Payment Reference Number. I also do not have access to my Employer ID Number.

All of the five choices need the CRN/SS Number.

But the good thing with UMID Card is that you have an option to either enter the UMID Card Pin Code which must be activated in the SSS Information Terminal located at SSS branches or your Mother's Maiden Name.
